“Money Follows the Person Demonstration: Illinois Pathways to Community Living Program “


~~“This notice provides important information regarding the Money Follows the Person Demonstration: Pathways to Community Living (MFP Pathways) Program’s referrals and transitions. 
MFP Pathways was developed under the Money Follows the Person Rebalancing Demonstration, which was authorized by the Federal Deficit Reduction Act of 2005 and extended under Section 2403 of the Affordable Care Act. MFP Pathways supports state long term care rebalancing by assisting eligible individuals to move from qualifying long term care facilities to community settings.
MFP Pathways referrals will no longer be accepted after June 30, 2017. MFP Pathways transitions to community settings will not be initiated after December 31, 2017. MFP Pathways participants who transition to the community before December 31, 2017 will receive 365 days of post-transition services. 

Although the MFP Pathways program is ending, Illinois remains committed to rebalancing the Long Term Services and Supports (LTSS) system. To facilitate the identification and assessment process of institutional residents interested in returning to the community, the current online MFP referral webpage will remain operational and supported by Healthcare and Family Services after June 30, 2017. “
