~~“Horizon Housing Communities Development for Ohio was awarded a statewide 2019 CMS Navigator Cooperative Agreement serving Young people aging out of their parents’ insurance; the working the poor; those that are “left behind”—made up of two separate contingency clusters: young adults reaching age 26 whose parents have included them on their private insurance coverage (or who do not have coverage through student or employer plans), and children reaching age 18 who are losing Medicaid insurance coverage either because they are the children of TANF eligible parents or have been in the foster care or adoption system and were covered by mandate. There are no Sub-awardee/Subrecipient Contracted Organizations. They will partner with County One-Stop Centers/Departments of Job and Family Service, Local Community Action Partnership, Cultural organizations, Libraries – branches throughout the target area, Faith-based organizations, Grassroots community based providers, Volunteer Match, Network for Good, and Idealist.org.  For more information, please contact the designated project lead.
